What is Brain Stroke ?

Brain cell function requires a constant delivery of oxygen and glucose from the bloodstream. A stroke, or cerebrovascular accident (CVA), occurs when blood supply to part of the brain is disrupted, causing brain cells to die. Blood flow can be compromised by a variety of mechanisms. Brain stroke is of two types:

Blockage of an Artery in the brain:- In this kind of stroke which is known as ischemic stroke, the blood supply to some part of the brain is stopped due to a blockage in the artery caused by atherosclerosis or deposition of cholesterol in the artery. In this kind of stroke the patient shows symptoms of paralysis.

Rupture of an artery (hemorrhage): This kind of stroke more commonly known as `Brain Hemorrhage' in common parlance is the situation when a artery carrying blood to the brain ruptures causing blood spill in the brain.

What are the Signs and Symptoms of Stroke ?

A stroke results from impaired oxygen delivery to brain cells via the bloodstream. According to the U.S. National Institute of Neurological Disorders and Stroke, the five major signs of stroke are the sudden onset of 

Numbness or weakness of the face, arm or leg, especially on one side of the body. The loss of voluntary movement and/or sensation may be complete or partial. There may also be an associated tingling sensation in the affected area.

Confusion, trouble speaking or understanding. Sometimes weakness in the muscles of the face can cause drooling.

Trouble seeing in one or both eyes

Trouble walking, dizziness, loss of balance or coordination

Severe headache with no known cause

 

Overall, the most common risk factors for stroke are:

- high blood pressure
- high cholesterol
- smoking
- diabetes
- increasing age.

Heart rhythm disturbances like atrial fibrillation, patent foramen ovale, and heart valve disease can also be the cause. When strokes occur in younger individuals (less than 50 years old), less common risk factors to be considered include illicit drugs, such as cocaine or amphetamines, ruptured aneurysms, and inherited (genetic) predispositions to abnormal blood clotting.An example of a genetic predisposition to stroke occurs in a rare condition called homocystinuria, in which there are excessive levels of the chemical homocystine in the body. Scientists are trying to determine whether the non-hereditary occurrence of high levels of homocystine at any age can predispose to stroke


An MRI (Magnetic Resonance Imaging) is the gold standard for diagnosing stroke. The MRI can diagnose a stroke as early as 45 minutes from the onset. If the patient cannot be taken for MRI then a good alternative will be CT Scan of the Brain. Most of the cities do have the facility of CT Scan and Neurologists who would be treating the stroke. In metro towns the patient should be rushed immediately to a stroke centre which is equipped with the best MRI machine and backed by a team of neurologists. Time is brain in the case of brain stroke.

Thrombolysis - Tissue plasminogen activator (TPA) - There is opportunity to use alteplase (TPA) as a clot-buster drug to dissolve the blood clot that is causing the stroke. There is a narrow window of opportunity to use this drug. The earlier that it is given, the better the result and the less potential for the complication of bleeding into the brain. Present American Heart Association guidelines recommend that if used, TPA must be given within 4 1/2 hours after the onset of symptoms. for patients who waken from sleep with symptoms of stroke, the clock starts when they were last seen in a normal state.
